Plus de 100 scientifiques, médecins et principales autorités appellent à une utilisation accrue de la vitamine D pour lutter contre le COVID-19 !

Des preuves scientifiques indiquent que la vitamine D réduit les infections et les décès.

À tous les gouvernements, responsables de la santé publique, médecins et travailleurs de la santé,

La recherche montre que de faibles niveaux de vitamine D favorisent les infections, les hospitalisations et les décès par COVID-19. Compte tenu de son innocuité, nous appelons à une augmentation des apports en vitamine D.

La vitamine D module des milliers de gènes et de nombreux aspects de la fonction immunitaire, à la fois innée et adaptative. Les preuves scientifiques [1] montrent que :

  • Des taux sanguins plus élevés de vitamine D sont associés à des taux inférieurs d’infection par le SRAS-CoV-2.
  • Des niveaux de D plus élevés sont associés à un risque plus faible de cas grave (hospitalisation, soins intensifs ou décès).
  • Les études d’intervention (y compris les essais cliniques contrôlés randomisés) indiquent que la vitamine D peut être un traitement très efficace.
  • De nombreux articles révèlent plusieurs mécanismes biologiques par lesquels la vitamine D influence le COVID-19.

La modélisation de l’inférence causale, les critères de Hill, les études d’intervention et les mécanismes biologiques indiquent que l’impact de la vitamine D sur le COVID-19 est très probablement causale, et pas seulement une corrélation.

La vitamine D est bien connue pour être essentielle, mais la plupart des individus n’en consomment pas assez. Deux définitions courantes de l’insuffisance sont une carence <20 ng / ml (50 nmol / L), cible de la plupart des organisations gouvernementales, et une insuffisance <30 ng / ml (75 nmol / L), la cible de plusieurs sociétés médicales et experts [2].

Trop de gens ont des niveaux inférieurs à ces objectifs. Les taux de carence en vitamine D (<20 ng / ml) dépassent 33% de la population dans la plupart des pays du monde, et la plupart des estimations d’insuffisance (<30 ng / ml) sont bien supérieures à 50% (mais beaucoup plus élevées dans de nombreux pays). [3] Les taux sont encore plus perturbés en hiver, et plusieurs groupes présentent une carence nettement plus importante : les personnes en surpoids, celles à peau foncée (surtout loin de l’équateur) et les résidents des maisons de soins. Ces mêmes groupes font face à un risque accru de COVID-19.

Il a été démontré que 3875 UI (97 mcg) par jour sont nécessaires pour 97,5% des personnes pour atteindre 20 ng / ml, et 6200 UI (155 mcg) pour 30 ng / ml [4], apports bien au-dessus de toutes les directives nationales. Malheureusement, le rapport qui a établi le RDA américain a inclus une erreur statistique admise dans laquelle l’apport requis a été calculé comme étant environ 10 fois trop faible [4]. 

Maintenant, de nombreux articles indiquent qu’un faible statut en vitamine D affecte le COVID-19 plus fortement que la plupart des autres problèmes de santé, avec un risque accru à des niveaux <30ng / ml (75nmol / L) et un risque sévèrement plus élevé <20ng / ml (50nmol / L) [1].

Les preuves à ce jour suggèrent la possibilité que la pandémie de COVID-19 se maintienne en grande partie par l’infection de ceux qui ont un faible taux de vitamine D, et que les décès sont principalement concentrés chez ceux qui souffrent de cette carence. La simple possibilité qu’il en soit ainsi devrait obliger à rassembler d’urgence davantage de données sur la vitamine D. Même sans plus de données, la prépondérance des preuves indique qu’une augmentation de la vitamine D aiderait à réduire les infections, les hospitalisations, les admissions aux unités de soins intensifs et les décès.

Des décennies de données de sécurité montrent que la vitamine D présente un risque très faible : la toxicité serait extrêmement rare avec les recommandations présentées ici. Le risque de niveaux insuffisants l’emporte de loin sur tout risque lié à des niveaux qui semblent fournir la majeure partie de la protection contre le COVID-19, et ceci est particulièrement différent des médicaments et des vaccins. La vitamine D est beaucoup plus sûre que les corticoïdes, tels que la dexaméthasone, le traitement le plus largement accepté pour avoir également démontré un grand avantage du COVID-19. La sécurité de la vitamine D ressemble plus à celle des masques faciaux. Il n’est pas nécessaire d’attendre d’autres essais cliniques pour augmenter l’utilisation de quelque chose d’aussi sûr, en particulier lorsque remédier à beaucoup de carences ou d’insuffisances devrait déjà être une priorité.

Par conséquent, nous appelons tous les gouvernements, médecins et professionnels de la santé du monde entier à recommander et à mettre en œuvre immédiatement des efforts adaptés à leurs populations adultes, pour augmenter la vitamine D, au moins jusqu’à la fin de la pandémie. Plus précisément pour :

  • Recommander des quantités de toutes les sources suffisantes pour atteindre des taux sériques de 25(OH)D supérieurs à 30 ng / ml (75 nmol / L) , un minimum largement approuvé avec des preuves d’un risque réduit de COVID-19.
  • Recommander aux adultes un apport en vitamine D de 4000 UI (100 mcg) par jour (ou au moins 2000 UI) en l’absence de test. 4000 UI sont largement considérés comme sûrs. [5]
  • Recommander que les adultes présentant un risque accru de carence en raison d’un excès de poids, d’une peau foncée ou vivant dans des maisons de soins puissent avoir besoin d’un apport plus élevé (par exemple 2 fois plus). Les tests peuvent aider à éviter des niveaux trop bas ou trop élevés.
  • Recommander aux adultes qui ne reçoivent pas déjà les quantités ci-dessus de recevoir 10 000 UI (250 mcg) par jour pendant 2-3 semaines (ou jusqu’à atteindre 30 ng / ml en cas de test), suivi de la quantité quotidienne ci-dessus. Cette pratique est largement considérée comme sûre. Le corps peut synthétiser plus que cela à partir de la lumière du soleil dans les bonnes conditions (par exemple, une journée d’été à la plage). En outre, le NAM (États-Unis) et l’EFSA (Europe) le qualifient tous deux de «niveau sans effet indésirable observé», même en tant qu’apport d’entretien quotidien.
  • Mesurer les taux de 25(OH)D de tous les patients hospitalisés COVID-19 et traiter avec du calcifediol ou de la D3, pour au moins remédier à une insuffisance <30 ng / ml (75 nmol / L), éventuellement avec un protocole du type de Castillo et al. ou Rastogi et al, jusqu’à ce que des preuves soutiennent un meilleur protocole.
  • De nombreux facteurs sont connus pour prédisposer les individus à un risque plus élevé d’exposition au SRAS-CoV-2, tels que l’âge, le fait d’être un homme, les comorbidités, etc., mais une vitamine D inadéquate est de loin le facteur de risque modifiable le plus facilement et le plus rapidement avec de nombreuses preuves.
  • La vitamine D est peu coûteuse et présente un risque négligeable par rapport au risque considérable de COVID-19.
